PLEASE READ THIS DOCUMENT CAREFULLY BEFORE AGREEING. IT IS A LEGAL DOCUMENT THAT AFFECTS YOUR RIGHTS.

1. Assumption of Risk

I, the agreeing participant, acknowledge that participating in any dance-related activity, including but not limited to the Event involves inherent risks of physical injury. These risks include, but are not limited to, falls, collisions, sprains, strains, fractures, dislocations, concussions, cuts, bruises, and other serious injuries, including permanent disability or death. I understand that these risks may arise from my own actions or inactions, the actions or inactions of others, or the condition of the facilities or equipment.

I affirm that I am in good physical condition and do not suffer from any known disability or medical condition that would prevent or limit my participation in this competition or that would be exacerbated by my participation. I acknowledge and agree that I am voluntarily participating in the Event with full knowledge of these potential risks and dangers. I assume full responsibility for any and all risks of injury, death, or property damage suffered by me while participating in this event.

2. Waiver and Release of Liability

In consideration of being permitted to participate in the Event, I, for myself, my heirs, executors, administrators, personal representatives, successors, and assigns, hereby waive, release, discharge, and covenant not to sue the Organizations, their officers, directors, employees, volunteers, agents, sponsors, and the owners and lessors of the premises where the Event is held (collectively, the "Released Parties"), from any and all claims, demands, liabilities, actions, or causes of action of any kind, whether known or unknown, foreseen or unforeseen, arising out of or relating to any loss, damage, injury (including death), or property damage that I may sustain or incur as a result of my participation in the Event, even if caused by the negligence or fault of the Released Parties.

I expressly agree that this waiver and release is intended to be as broad and inclusive as permitted by the laws of the State of Texas and that if any portion thereof is held invalid, the balance shall, notwithstanding, continue in full legal force and effect.

3. Medical Treatment Consent (and refusal)

I understand that the Organizations are not responsible for providing medical insurance or medical care for participants. I certify that I have adequate health insurance coverage to cover any injury or damage I may suffer while participating in this event, or I agree to bear the costs of such injury or damage myself.

In the event of an emergency, I authorize the Released Parties, or their representatives, to obtain medical attention for me, if necessary, and I agree to be responsible for any and all costs associated with such medical care, treatment, or transportation. I understand that this authorization does not impose a duty upon the Released Parties to provide medical treatment.

4. Media Release / Use of Image

I I grant the Organizations, and their designees, the irrevocable right and permission to copyright, use, re-use, publish, and republish photographs, videos, audio recordings, or other media of me, in whole or in part, individually or in conjunction with other images or videos, and in any medium (including, but not limited to, print, internet, social media, and broadcast) and for any purpose whatsoever, including illustration, promotion, advertising, and trade, without restriction as to alteration.

I hereby waive any right to inspect or approve the finished product or the advertising copy or other matter that may be used in connection therewith, or the eventual use to which it may be applied. I also waive any right to royalties or other compensation arising from or related to the use of my image or likeness.

5. Participant Affirmations

I affirm that I am 18 years of age or older and have the legal capacity to enter into this agreement. I have read this Waiver and Release of Liability and Media Release Form, fully understand its terms, and I agree to it freely and voluntarily without any inducement. I understand that by agreeing to this document, I am giving up substantial legal rights, including the right to sue.

6. Governing Law and Jurisdiction

This Waiver and Release shall be governed by and construed in accordance with the laws of the State of Texas. Any legal action or proceeding arising out of or relating to this Waiver and Release shall be brought exclusively in the courts of Dallas County, Texas.

7. Final Affirmation

By agreeing, I acknowledge that I have read, understood, and agree to all the terms and conditions contained in this entire Waiver and Release of Liability and Media Release Form. I understand that this is a legally binding document and that I am waiving certain legal rights.
